Paperwork needed for Prescription Advantage enrollment

Please bring the following with you to your appointment or supply electronically: 

  • Photo ID, that includes proof of address

  • Medicare and any Secondary Health Insurance cards 

  • Proof of Income for all members of the home:

    • SSI

    • Pension Check

    • Retirement Annuity

    • Paycheck

    • Rental Income

  • If you file federal income taxes, your most recently filed federal income tax return 

  • Your most recent 1099 or W-2 form(s) for each type of income listed below: 

    • Pensions/Annuities 

    • Dividends/Interest 

    • Retirement (IRA, 401K, 403B) 

    • Rental Income 

    • Capital Gains 

    • Employment Income 

    • Alimony  

    • Railroad Benefit Income
