Paperwork needed for Medicare Savings Program enrollment

Please bring the following with you to your appointment or supply electronically: 

  • Photo ID, that includes proof of address  

  • Medicare and any Secondary Health Insurance cards  

  • Proof of all income 

    • SSI; 

    • Pension Check;  

    • Retirement Annuity;  

    • Paycheck;  

    • Rental Income
