Paperwork needed for MassHealth enrollment

Please bring the following with you to your appointment or supply electronically: 

  • Photo ID, that includes proof of address

  • Medicare and any Secondary Health Insurance Cards 

  • Two months of all bank statements 

  • Proof of all sources of income:

    • SSI

    • Pension Check

    • Retirement Annuity

    • Paycheck

    • Rental Income

  • Life Insurance (1st page only) 

  • Deed or Mortgage Statement 

    • Property Tax Bill 

    • Homeowner’s Insurance Bill

  • Proof of any: 

  • Stocks

  • Bonds

  • Mutual Funds 

  • Annuities

  • Trusts 

  • Car Registration 

  • Contract of deposit to any healthcare facility or long-term residential facility 

  • Previous three months of medical expenses if you plan to claim them
